Covid-19 Self-declaration Form

Passengers will have to submit a self-declaration form on the online Air Suvidha portal and include travel history for the past two weeks. They will also be required to upload a negative COVID-19 RT-PCR test report.

Delhi High Court sought response of the Centre and aviation body DGCA on a plea seeking to do away with the mandatory online self-declaration form which has to be filled up by the passengers before boarding a flight that they are Covid negative.

JEE Main Admit Card: The hall ticket consists of three pages- page 1 contains the details of examination centre and self-declaration (undertaking) form regarding COVID-19, page 2 has “important instruction for candidates", and page 3 has “advisory for candidates regarding COVID-19".

International passengers arriving at Delhi airport can fill a mandatory self-declaration form online. This will make travelling contactless reduce congestion at the arrival lounge. From Saturday onward, five categories of passengers can get exemption from the mandatory seven-day institutional quarantine.
